Once you have read all the information and you understand the policies, please fill out the

PDF Application Form/Contract

  • All applications must be approved before you will be notified of a booking date.
  • All new vendor applications will be placed on a waiting list and you will be notified when an opening occurs.
  • All bookings must be confirmed by mailing your rent cheque/s post-dated for your booked date/s.
  • If you are booked to come in on a “last minute” cancellation, please bring your rent cheque with you when you arrive.

    Upon arriving “day of” your booked date, you will enter the hospital through the back area of Shipping and Receiving between the hours of 7:30 am – 8:30 am. There will be carts/dollies available in Shipping/Receiving for you to conveniently move your product to your kiosk space. You are allowed to stay as long as you wish that day but please note Shipping/Receiving Department closes at 4:00 pm and you will have to contact hospital Security if you wish to exit later than that time.
